Information for New Patients
During your first visit to our office, we will begin by asking your to fill out our new patients forms. You can download and fill out the forms ahead of time, or arrive approximately 10 minutes early for your appointment, and fill them out at our office.
We will conduct a thorough dental exam to establish a baseline for your oral health, and plan any future dental treatment. Depending on your needs and the scheduled time, we may perform a dental cleaning during your first appointment as well.
Please note: We require two business days notice to cancel or re-schedule an appointment without incurring a charge.
If you are a new patient to our office, the attached file contains our new patient bundle with forms that will need to be filled out when you arrive at our office. Save some time by printing them and filling them out at home and bringing them in the day of your appointment.
New Patient Form Safety Protocols
For your convenience, we accept cash, debit card (INTERAC), money orders or VISA/Mastercard. Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office.
We also accept payment through a third party financing company called Dentalcard, a division of iFinance. You can read more about their program at www.dentalcard.ca.
We accept all dental insurance policies and direct bill your insurance company for your convenience. You would only be responsible for your co-payment at the time of service, and then have the insurance company reimburse us for the balance.
